This is the COVID-19 survey for Pleasant View Elementary. Please complete the survey for all students attending PVES. Complete the information on one student at a time. When the first student is complete, you will receive a prompt that states would you like to add an additional student. If so, click and complete the survey for the additional students at PVES. If no other children, click complete. If you have students in another school, a school specific survey will be sent from that school.
